As an RN Geriatric Care Manager, I assist clients and families with coordination of all care and services needed to maintain a patient’s safety, independence, and well-being, as well as maintaining cost effective options. This includes medication management, understanding the need for medical equipment or supplies ( bed, wheelchair, oxygen, hoyer lift, shower assistive devices, commode, stairlift), physical and occupational therapy, home safety and fall prevention assessment, joint medical visits at the patient’s personal physician, and hospice care, should it become necessary.
